YOUR INFORMATION
Privacy, in plain language.
This notice covers the initial ChinaMed coordination enquiry form. Updated 12 October 2026.
What the form collects
We collect your name, email address, country or region of residence, preferred language, city, travel timing, the kind of support you select and your consent. We also save a reference number, submission time, enquiry status and a basic referral-source category if provided in the link you used.
The form does not ask for diagnoses, medical records, passport details or payment information. Please do not enter those details into the available fields. For someone else’s trip, use your own contact information.
Why we use it
The information lets the ChinaMed coordination team review your enquiry, understand the practical support being considered and contact you about that request. Consent to this form does not subscribe you to marketing.
Storage and access
Enquiries are stored in the site’s hosted database. The staff inbox is restricted to the authorised operator account. Hosting services process the data to run the website. An enquiry is not automatically sent to a hospital or interpreter.
Enquiries remain in the inbox until the operator deletes them. This initial version does not automatically expire enquiries. If a later service requires information to be shared with a provider, the recipient, purpose and appropriate consent must be agreed separately.
Cookies and tracking
ChinaMed does not install advertising pixels or store form details in your browser’s local storage. The hosting platform may use necessary security and sign-in cookies. Staff sign-in is separate from the enquiry form.
Request deletion
You can submit a data deletion request using the email address associated with your original enquiry. The team must verify that you are entitled to act on the record before deleting it. The request form records this request; it does not instantly erase data.
If you are already in contact with a coordinator, you can also ask them to correct or delete your enquiry details. Keep your reference number for that conversation.
Before a paid service
The formal service agreement must identify the contracting provider and its contact details, as well as any additional privacy arrangements needed for that service. This form does not authorise the collection or transfer of medical records.
